Quoting and Appointing Us to Your Scheme

To appoint Lime Street Insurance Brokers as your advisor or broker, please complete the following:

  • AMII Form – Indicates your choice of advisor for health insurance. Complete this if you require advice from us or wish to change your current broker.
  • Letter of Appointment – Authorizes us to manage your group scheme. We will submit this to the insurer for processing.
  • Employee Data Form – Provides the necessary employee details to obtain quotes for all benefit types listed above.

Please note: Your insurer may contact you to confirm your instructions and, where appropriate, notify your current intermediary.
